Common Application College Report Instructions

The Office of the Registrar and Office of Student Conduct and Conflict Resolution (OSCCR) are the only offices permitted to complete the College Report.

Northeastern University students planning to transfer to another school which utilizes the Common Application should follow the instructions below to have the College Report completed. The Office of the Registrar and Office of Student Conduct and Conflict Resolution (OSCCR) are the only offices permitted to complete the College Report.

1. Complete and sign the top half of the form.
2. Send the form to the Transcript Office. Include your NUID number and complete the address information for all school(s) into which you are applying to transfer.
3. Email the form to [email protected]

Transcript Requests

The Registrar’s Office will complete its sections of the College Report and the form will be forwarded to the Office of Student Conduct and Conflict Resolution (OSCCR). OSCCR will then mail the completed College Report to the school(s) of address given.
